LA ESTRATEGIA DIAGNOSTICA DEL ADMINISTRATIVO DE URGENCIAS

Paciente con clínica de cólico nefrítico solicitamos tira reactiva de microhematuria y es negativa, y la radiografía no se ve la litiasis. Entonces ¿ que hacemos? Cuando empece a trabajar en urgencias me dijeron los lugareños que cuando el paciente pasaba y el administrativo le decía al celador: “ pasalo es un colico al riñon” esto tenía una alta fiabilidad. Como se ve la sensibilidad de la RX es baja y tiene un elevado numero de falsos negativos que conlleva a que el valor predictivo negativo del tests sea también bajo con lo que un test negativo no descarta el cólico de nuestro paciente. El mismo dato es aplicable a la tira reactiva por lo que deberemos apoyarnos en la clínica a la hora de definir nuestra actuación.
